Please Note: ****Proof of Vaccination Steps listed below****

As a registered student for our course, you will receive an email from STCC (Sender should be STCC, with a subject line of “Proof of Vaccination Submission Steps”) with a link for Dynamic Forms. 

After Receiving Your Email From STCC:

1. Click on the Dynamic Forms link.
2. Dynamic Forms will ask you to create a basic account and verify your email address.
3. Once you verify your email; you will be able to log back into the portal and upload your vaccination card.
4. Please note: There is a student ID section that has to be filled in- just input the number (i.e #9999999) that appears next to the box.

IMPORTANT: If you registered for class after January 4- your registration information just went over to STCC yesterday; therefore, please allow a few days to receive your email from them with the Dynamic Forms link. Otherwise, you should have already received the email. Please be sure to check your spam folder if you don’t see it. 

REGARDLESS: Please plan to bring your vaccination card with you to campus- even if you’ve uploaded it with Dynamic Forms. Additionally, print or save confirmation with Dynamic Forms.

ALL SIX (6) OF THESE BOOKS ARE REQUIRED FOR THIS COURSE

Mask Advisory for Vaccinated and Unvaccinated Residents
****Effective December 21, 2021: In response to the spread of the Delta variant and the emerging Omicron variant, the Department of Public Health now advises that all residents, regardless of vaccination status, wear a mask or face covering when indoors (and not in your own home). The DPH particularly urges this recommendation if you have a weakened immune system, or if you are at increased risk for severe disease because of your age or an underlying medical condition, or if someone in your household has a weakened immune system, is at increased risk for severe disease, or is unvaccinated. ****
